Understanding Question Types, Linked Annotations, and Conditional Visibility

Maximize the effectiveness of your feedback form by using a variety of question types, linked annotations, and conditional visibility to create dynamic and personalized forms.

What question types are possible in the feedback form?

  • Single Line Answer
    The response is kept to a single line.

  • Multiline Answer
    The text can span multiple lines.

  • Dropdown
    A list of options can be added in a dropdown menu.
    Note: Only one option can be selected as the answer.

  • Multiple Choice
    One option is required as a response, but users can select multiple options if allowed.

Linked Annotations

You can link annotations to specific answers in the feedback form to provide further context or detail based on user responses.

Conditional Visibility

The feedback form builder supports conditional logic, meaning the flow of questions can be determined by previous answers. This allows for more personalized and relevant feedback collection.

How Does Conditional Visibility Work?

  1. Start by creating a review form.

  2. Add your first question.

    • Example: In a call from a patient to a weight loss specialist, the first question might be, "Is this the first call or a follow-up?"

  3. Click Add New Field to insert the next question. Now, the Set Visibility Condition option will be available for the second question.

    • If it’s the first interaction, the call taker should ask health history-related questions. Set the visibility condition accordingly.

    • Result: If the answer to the first question is Yes, then the second question might be, "Has the previous health history been addressed?"

Why customize your Question Flow

One question can have multiple visibility conditions to align with the review process in the most efficient way, enabling complex workflows and tailored feedback. This allows for different levels of detail to be gathered based on the variable requirements of the call type or content.
