Using the feedback form template builder to create templates

Mission Control’s feedback forms play a key role in enabling robust QA practices and fostering an effective feedback loop. Follow the steps below to create a new feedback form:

How to Create a New Feedback Form

Navigate to the Templates Page
Go to the Templates section located within Organization Settings.

Create or Duplicate a Template

  • Click on Create New Template to start from scratch.

  • Alternatively, you can choose an existing template and click Duplicate to use it as a base for your new form.

Name and build your form

Begin by adding a title for your feedback form and then add your questions. Mission Control supports multiple question types, including:

  • Single-line answer

  • Multi-line answer

  • Drop-down selection

  • Multiple choice

Associate Phrases or Annotations
You can link specific phrases or annotations to an answer to provide more detailed feedback based on user input.

Add Conditional Questions
Use conditional logic to display certain questions only based on the answers given to previous questions, allowing for more tailored feedback forms.

Review and Publish Your Form
Once all questions have been added, thoroughly review the form. When you're satisfied, click Publish to make it available for use.

Test Your Form
To ensure everything works as expected, navigate to Single Case View and test your form on a call.
